Bio-Medical Library E-mail Renewal Form

Use this form to request renewals of loans from the Bio-Medical Library. Be sure that requests are made on or before the due date to avoid overdue fines. Requesting a renewal for a non-renewable item will not prevent fines from being assessed. Only 2 renewals on the same loan are allowed under standard library policy and by our automated circulation system. Renewals are not available on overnight or reserve items.

This is NOT an automatic renewal process, it is only a form to compile information that will be E-mailed to library staff. Bio-Medical Library staff will later manually enter your request into the automated system within several days and e-mail a response to you. For more immediate action and a response, we suggest telephoning 612-626-5653 or 612-626-4045.

This form is only for materials owned by the Bio-Medical Library. Renewals from other University of Minnesota Libraries can be requested on another similar form. Note that if you request Bio-Medical renewals on that form you will likely delay the processing of your request until it can be forwarded to Bio-Medical Library.

--note: this is NOT your student i.d. number. On a U card it appears next to the label "library" and typically begins something like "2UCSA. . ." and is 15 digits long.

First Item to be renewed:

*Title (book, or journal volume, or other material):
*Barcode

--note, this number should begin "31951. . ." and is 15 digits long--please omit the spaces in entering this number. The barcode is not the call number, and usually is attached to the back cover of the book or volume, or directly on the videocassette. It is printed beneath the actual barcode.

*Call Number

--note, this number normally appears on the spine or front cover of a book. Journals do not have call numbers. Call numbers consist of several lines of letters and numeric digits.

-- OR -- *Journal Volume and Date

--journals do not have call numbers, so please supply the volume number and date, as for example: v. 25, 1996 normally this information appears on the spine of a bound volume, or the cover of an unbound issue.
